2. 請務必詳實填妥系統內各欄資料。
Please make sure to fill out the information needed for the application.
3. 繳費完成始受理申請。
Application will be accepted only after you pay the required application fee.
4. 請使用護照英文姓名,一經使用不得任意更改;如需更改請依下列辦理異動。
(1) 校友:請繳交 基本資料異動申請表 (請點我)
(2) 在校生:請於 教務系統填寫申請資料,詳見 操作手冊(請點我)
Please use the English name on your passport. Once the application has been submitted, the profile cannot be changed arbitrarily.
(1) Alumni: Please hand in "Application for Change of Personal Information".
(2) Current Student: Please log in the Academic Affairs Information System. For further information, please refer to the attachment "Operating manual".
5. 本系統無法同時接受兩個(含)以上學制之校友身分申請,請先擇一申請繳費至文件寄送或領取後,再至系統繼續其他身分之申請繳費。
Dual Enrollment Status can’t be accepted for application simultaneously in the system. Please choose one identity for application. After collecting your document or after the document has been mailed to the address you provide in the application, another application for the other Enrollment Status may be submitted.

7. 未經當事人同意代為申請各類文件者,如經告發,申請人須負相關法律刑責!
Applying for other people’s documents without their permission will be charged with a crime.
8. 若有以下情事,不予退費:
(1)有關需補件之申請單,經聯繫後一周內未收到回覆,即不製作、不予退費且無法更換同價品項。
(2)未符合該證明書資格者,例如未修畢微學程課程、學分學程等, 卻申請相關證明書, 不予退費亦無法更換同價品項
No refund will be given in the following cases:
(1) If not receive reply within one week after contacting the application for supplementary parts, documents will not be made. Application will not be refunded also cannot be changed to other items.
(2) Those who do not meet requirements of the certificate, such as not taking enough credits for mini program etc., but apply for relevant certificates, application will not be refunded and cannot be replaced with items of same price.
9. 本系統24小時開放申請,中午十二時後申請並完成繳費,視同隔日申請。取件日數以申請日起算。
領取時間(不含例假日):星期一至星期五,08:00-17:00
Service time of system is 24 hours. Apply and complete payment after 12 p.m. (GMT+8) will be deemed as application for next day. Working day of document will from next day.
Office opening for collecting documents: Mon.-Fri. 8:00 a.m.-5:00 p.m. (not includes holiday)
